	Medication: cefazoline iv one-off	Remarks: SWAB remarks laparoscopische cholecystectomie: alleen profylaxe bij omstandigheden met een verhoogd risico op gecontamineerde gal, zoals bij hoge leeftijd van de patiënt, acute cholecystitis of status na interventie aan de galwegen.
	Medication: cefazoline iv one-off + metronidazol iv 500mg one-off	Remarks: SWAB remarks dunnedarm- en colorectale chirurgie, appendectomie
		Remarks: SWAB remarks andere laparotomieën (schone ingrepen): geen profylaxe

STANDAARD DOSERING (15-60 min voor incisie)

- cefazoline iv 2000mg*, na 4 uur gift herhalen : cefazoline iv 1000mg†

- metronidazol iv 500mg, herhalen bij operatieduur > 8 uur

- clindamycine iv 600mg, herhalen bij operatieduur > 6 uur

* bij wondsluiting binnen 1 uur na toediening kan worden volstaan met cefazoline iv 1000mg

† bij resterende operatieduur > 3 uur cefazoline iv 2000mg

AANGEPASTE DOSERING OBESITAS

- cefazoline iv 3000mg (BMI > 40), na 4 uur gift herhalen : cefazoline iv 2000mg*

- metronidazol iv 500mg (geen aanpassing), herhalen bij operatieduur > 8 uur

- clindamycine iv 900mg (gewicht > 180kg), herhalen bij operatieduur > 6 uur

* bij resterende operatieduur > 3 uur cefazoline iv 3000mg

BIJ CONTRA-INDICATIE CEFAZOLINE, zie antibiotica allergie

clindamycine iv + gentamicine iv 6-7 mg/kg eenmalig
