General comments

Cholangitis: na adequate drainage van de galwegen: behandelduur maximaal 3 dagen

Altijd rekeninghouden met resistentie patronen van micro-organismen uit voorgaande kweken en met recent antibiotica gebruik.

Bij kolonisatie met ESBL in afgelopen jaar: meropenem 3 dd 1000 mg iv

Bij vertraagde penicilline-allergie (rash, geen anafylaxie): ceftriaxon iv 2000mg 1dd

Bij hemo-dynamisch instabiliteit niet reagerend op vulling: gentamicine 6-7 mg/kg iv 1dd toevoegen aan bovenstaand advies

Bij risicofactoren die gepaard gaan met een verhoogd risico voor ESBL*: gentamicine 6-7 mg/kg iv 1dd toevoegen aan bovenstaand advies

* recent (<2 maanden) gebruik cefalosporines en/of chinolone, ziekenhuisopname of hospital-acquired/health-care associated infectie
